Isolated right ventricular metastasis in a woman with advanced hepatocellular carcinoma after palliative therapy

Fan CT, et al. - As there are only a few case reports describing hepatocellular carcinoma (HCC) with cardiac metastasis, a group of rarity and poor prognosis, no clear information regarding the optimal treatment and predicted outcomes for these patients is available. Researchers here report on the case of a 67-year-old female patient presenting with dyspnea and heart murmur. The case was diagnosed with advanced HCC with isolated right ventricular metastasis. She was provided with the best supportive care due to the presence of multiple comorbidities regarding cardiac and pulmonary systems and reported to have survived 2 months after initial diagnosis. From PubMed, they identified 80 published cases (including this patient) since the 1950s and gathered the data from the medical record and literature reviews. This review revealed the involvement of the right heart mainly with a predominance of the right atrium (53%) among patients with advanced HCC and cardiac metastasis. This review indicated the overall 3-month survival rate of 70.7%. In subgroup analysis, patients treated with surgery and other therapies had the highest overall 3-month survival (97%), and patients with the best supportive care had the lowest overall 3-month survival (27%). They identified this work as the first comprehensive literature review inscribing the epidemiology, optimal treatment, and prognosis for advanced-stage HCC with cardiac metastasis.
