Is clinical exam of the axilla sufficient to select node-positive patients who downstage after NAC for SLNB? A comparison of the accuracy of clinical exam vs MRI
Annals of Surgical Oncology Oct 12, 2019
Moo TA, Jochelson MS, Zabor EC, et al. - The accuracy of postneoadjuvant chemotherapy (NAC) MRI was compared with that of clinical exam alone in predicting the pathologic status of sentinel lymph nodes in cN1 patients. Researchers grouped patients with T0-3, N1 breast cancer who underwent NAC and subsequent sentinel lymph node biopsy (SLNB), based on whether a post-NAC MRI was done. The final analysis included 269 patients; post-NAC MRI was performed in 68% (182/269). Outcomes revealed no higher accuracy of post-NAC MRI vs clinical exam alone in predicting SLN pathology in patients presenting with cN1 disease. SLNB should not be precluded in correlation with abnormal lymph nodes on MRI.
