Is a stepped-care intervention effective in overweight and obese people with medial tibiofemoral osteoarthritis? The STrEAMline study: A randomised controlled trial

Robbins SR, Melo LRS, Urban H, et al. - In this randomized controlled trial, researchers examined the efficacy of a 32-week stepped-care intervention on disease remission rates in overweight and obese people with medial tibiofemoral OA compared with control. Eligible candidates were ≥ 50 years-old with a BMI ≥ 28 kg/m2 and radiographic evidence of medial tibiofemoral OA. They randomized participants to stepped-care (n = 87) or control group (n = 84). Disease remission was reached in the control group at 32 weeks by 18 out of 68 and in the stepped-care group by 32 out of 82. The stepped-care group displayed pain and function improvement between baseline and 20-weeks. Although functional improvement was sustained at 32 weeks, between weeks 20 and 32, pain levels appeared to get worse. The proposed intervention did not promote a significant variation in the rate of disease remission compared with the control group for those with medial tibiofemoral OA who were overweight or obese.
