Invasive bacterial infections in afebrile infants diagnosed with acute otitis media

McLaren SH, Cruz AT, Yen K, et al. - Researchers performed a 33-site cross-sectional study to determine the prevalence of invasive bacterial infections (IBIs) and adverse events in afebrile infants with acute otitis media (AOM). The sample consisted of afebrile infants ≤ 90 days of age with AOM seen in emergency departments from 2007 to 2017. Using emergency department diagnosis codes, eligible infants were identified and validated by chart review. One thousand six hundred thirty-seven met research criteria out of 5,270 infants screened. Older infants were less likely to have received blood cultures or CSF cultures compared with infants 0 to 28 days old. There is a low prevalence of IBIs and adverse events in Afebrile infants with clinician-diagnosed AOM, so outpatient management without diagnostic testing may be appropriate.
