Ibrahim LF, et al. - Researchers performed this planned follow-up analysis of the Cellulitis at Home or Inpatient in Children from the Emergency Department (CHOICE) trial in order to ascertain the cost-efficacy of an admission avoidance pathway, in which children were treated at home, compared with standard hospital care for the intravenous treatment of moderate or severe cellulitis. One hundred eighty children (aged 6 months to 18 years) who comprised the per-protocol population in the CHOICE trial were included; 89 children received home treatment with intravenous ceftriaxone (the home group) and 91 children received hospital treatment with intravenous flucloxacillin (the hospital group). For the home group and the hospital group, the mean cost incurred per family was $182 and $593, respectively. The home group vs the hospital group had both measures of effectiveness significantly better: quality-adjusted life years were 0·005 for the home group vs 0·004 for the hospital group, and treatment failure occurred in one (1%) patient in the home group vs seven (8%) patients in the hospital group. Findings thereby support the superiority of treatment at home, in terms of efficacy as well as cost, when compared with standard hospital care for children with moderate or severe cellulitis. The development of this admission avoidance pathway in hospitals is thus supported by the findings.
