Intraoperative assessment of reduction quality during nail fixation of intertrochanteric fractures

Yoon YC, et al. - Researchers, for the first time, sought for criteria for the intraoperative evaluation of the quality of intertrochanteric fracture reduction and investigated postoperative outcomes in terms of reduction quality. They examined reduction quality using an image intensifier intraoperatively in 106 patients who received cephalo-medullary nailing for the treatment of intertrochanteric fractures and classified them into optimal, acceptable, and unacceptable groups based on the reduction quality. Active intervention by the surgeon was required in patients with Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association type 31A2 and A3 fractures. Traction alone cannot lead to good outcomes in femoral intertrochanteric fractures. These fractures require active management in view of the essentiality of accurate anteromedial cortex reduction, and restoration of neck-shaft angle and anteversion.
