Intra-abdominal hemorrhage following 739 consecutive pancreaticoduodenectomy: Risk factors and treatments

Lu JW, et al. - The researchers performed univariate and multivariate study to investigate the risk factors of post-pancreaticoduodenectomy hemorrhage (PPH) and to assess the treatment options. In this investigation, 739 consecutive patients undergoing pancreaticoduodenectomy between 2009 and 2017 were involved. No vital risk factor was identified in association with early PPH, however, pancreatic duct diameter <0.4cm, and intra-abdominal complications like pancreatic fistula, intra-abdominal abscess and delayed gastric emptying, might be linked to late PPH. The affected sites were confirmed by endoscopy, angiography, and/or exploratory laparotomy with grade B or C hemorrhage. It was concluded that hemorrhage following pancreaticoduodenectomy was a common and lethal complication. Treatment strategies should be suggested to patients according to distinct etiologies.
