Interobserver variability in breast carcinoma grading results in prognostic stage differences

Rabe K, et al. - Investigators sought to evaluate the degree of interobserver agreement for invasive carcinoma grade among pathologists and ascertain its impact on pathologic prognostic stage. Using established Nottingham grading criteria, a total of 100 consecutive cases of invasive stage II carcinomas were independently graded twice, with a 4-week intervening wash-out period, by 6 breast pathologists. This study ascertained inter- and intra-observer variability for overall grade and for each of the 3 scoring components. The outcomes of this study indicated that grading agreement between pathologists was good to very good and discordant grades occurred in small changes to the pathologic prognostic stage.
