Insufficient left ventricular unloading after extracorporeal membrane oxygenation

Hu W, et al. - In this review of a case series, researchers assessed the impacts of intra-aortic balloon pump (IABP) and beta-blockers for the treatment of left ventricular (LV) overload after extracorporeal membrane oxygenation (ECMO) in patients with fulminant myocarditis (FM). Of eight patients, six survived. The occurrence of insufficient LV unloading after ECMO treatment was reported in six patients. Electrical storm was experienced by three of these six patients but interventions with beta-blockers and IABP resulted in return of spontaneous circulation. Full recovery of cardiac function was observed in survivors. Overall, the occurrence of electrical storm may be prevented with beta-blockers, and it is feasible to use IABP to treat LV overload after ECMO application.
