Insomnia in community-living persons with advanced age

Miner B, et al. - Researchers assessed the epidemiology of insomnia, including demographic and clinical correlates, in older adult participants from Yale Precipitating Events Project (N=379; mean age 84.3; 67.8% female; 11.9% African American). In this study using cross-sectional design, they used Insomnia Severity Index (ISI) in a sample of older adults with high medical burden and polypharmacy and found a high prevalence of insomnia in these subjects, though it was unexpectedly mild and showed an association only with depressive symptoms and restless legs syndrome (RLS). There is a need for diagnostic corroboration with objective measures of sleep disturbances, as suggested by the observed discordance of high prevalence but mild severity of insomnia in the oldest adults, while the strong correlations with depressive symptoms and RLS informed priorities in managing insomnia.
