Initial postoperative hemoglobin values and clinical outcomes in transfused patients undergoing noncardiac surgery

Will ND, Kor DJ, Frank RD, et al. - Researchers examined the links between initial postoperative hemoglobin values and a primary outcome of hospital-free days, using multivariable regression analyses adjusting for patient illness, laboratory derangements, and surgical features, among adults receiving allogeneic red blood cells during noncardiac surgery in this single-center observational cohort study. Superior outcomes were achieved in transfused patients in correlation with postoperative hemoglobin values between 7.5 and 11.5 g/dL vs more extreme values. For intraoperative transfusions, this range may be viewed as a target, especially during active bleeding when pretransfusion hemoglobin thresholds may be impractical or inaccurate. It may be preferable to target hemoglobin at the lower aspect as similar outcomes were seen within this range.
