Initial diagnosis of insignificant cancer, HGPIN, ASAP and negative have the same rate of upgrade to Gleason ≥7 on repeat prostate biopsy

Prathibha S, et al. - Considering the utility of knowledge of biopsy characteristics indicating risk for progression to guide therapeutic management, researchers reviewed prostate needle core biopsies performed between 2008–2014. Outcomes suggested that insignificant cancer, high grade prostatic intraepithelial neoplasia (HGPIN), atypical small acinar proliferation (ASAP), and negative upgraded at similar rates. Significant vs insignificant Gleason ≤6 were noted to have a greater rate of upgrade to Gleason ≥7. A higher rate of upgrade to Gleason ≥7 was noted in cases with >1 vs 1 core positive.
