Indications for fluid resuscitation in patients with septic shock: Post-hoc analyses of the CLASSIC trial

Bjerregaard MR, et al. - Researchers performed a post hoc analysis of the CLASSIC trial, which included patients admitted to an intensive care unit (ICU) with septic shock, to determine the indications currently used for fluid resuscitation, as well as the influence of fluid on these indications. Fluid restriction or standard care was randomly given to patients with septic shock. During the first 24 hours, a record was kept of indications for, and impact of, each fluid bolus. In the standard care group, 76 patients received 256 fluid boluses on 515 indications. The indications for fluid resuscitation, which were most frequently noted, included low blood pressure, high lactate, and low urinary output. The impact of fluids when offered on these indications were less clear, but may depend on the time course of sepsis resuscitation.
