Incremental value of renal dysfunction addition to the CHA2DS2-Vasc score for mortality prediction in patients with acute coronary syndrome

Castini DC, Persampieri S, Sabatelli L, et al. - Whether CHA2DS2-VASc score has utility for predicting mortality in patients suffering from acute coronary syndromes (ACSs), was assessed herein. Also, it was investigated whether its predictive accuracy could be improved by the addition of renal functional status. This study involved 908 ACS patients (median age 68 years, 30% female, 51% ST-elevation). Of these, 871 were discharged, and among those, 865 (99%) completed a 12-month follow-up. Researchers computed CHA2DS2-VASc score by employing both the original scoring system and adding renal functional status using 3 alternative renal dysfunction definitions. Findings showed a good predictive accuracy of CHA2DS2-VASc score for inhospital mortality as well as post-discharge mortality in ACS patients. An improved identification of cases at risk of death may potentially be achieved with the addition of renal dysfunction to the original score.
