Increased posttransplant mortality for autoimmune hepatitis compared with other autoimmune liver diseases

Lee JY, et al. - In the present study, researchers compared graft survival and the need for retransplant in autoimmune hepatitis (AIH) vs other autoimmune liver disease. They compared posttransplant graft survival among the three entities utilizing Cox regression and competing for risk analyses using the United Network for Organ Sharing (UNOS) database. After controlling for potential confounders, AIH is related to significantly reduced graft survival vs primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC). This is driven primarily by death from posttransplantation. Findings suggested an association of AIH with higher risk of death vs PBC and PSC and lower risk of retransplant vs PSC on competing for risk analysis. As previous studies have shown a comparable risk of recurrence of disease in AIH and PSC, the research shows that at least part of the increased posttransplant mortality in AIH may be due to lower retransplantation rate in this population.
