Increased fracture risk in women treated with aromatase inhibitors vs tamoxifen: Beneficial effect of bisphosphonates

Pineda-Moncusí M, Garcia-Giralt N, Diez-Perez A, et al. - Researchers conducted an observational cohort study up to 10 years of follow-up to assess the fracture risk in breast cancer patients receiving aromatase inhibitors, compared with tamoxifen users, and to evaluate the efficacy of oral bisphosphonates in decreasing fracture risk. According to low (without osteoporosis diagnosis nor bisphosphonates exposure) or high (with osteoporosis and/or treated with bisphosphonates) fracture risk, women diagnosed with breast cancer between 2006 and 2015 and treated with tamoxifen or aromatase inhibitors (n = 36,472) were stratified. During aromatase inhibitor treatment, fracture risk in postmenopausal women, in real-life conditions, was > 40% in comparison with tamoxifen, authenticating former randomized controlled trial outcomes. In high-risk individuals, bisphosphonate users vs non-bisphosphonate-users had lower notable fracture incidence during aromatase inhibitor therapy. Moreover, in aromatase inhibitor individuals, monitoring fracture risk and related risk factors are suggestive.
