Incident diabetes in survivors of critical illness and mechanisms underlying persistent glucose intolerance: A prospective cohort study

Kar P, et al. - In this prospective, single-center, cohort study, investigators assessed 40 subjects with a mean age of 58 years to understand the underlying mechanism of incident diabetes and prevalent prediabetes in survivors of critical illness encountering stress hyperglycemia. They measured the hemoglobin A1c during admission to ICU as well as 3 and 12 months later with oral glucose tolerance testing and hormone concentrations. Gastric emptying was also estimated through an isotope breath test. About 37% of the total had diabetes whereas 43% were prediabetics, at 3 months. The frequent occurrence of diabetes and prediabetes in survivors of critical illness experiencing stress hyperglycemia should warrant structured screenings and intervention programs in this population.
