Incidence rate of gastric cancer adenocarcinoma in patients with gastric dysplasia: A systematic review and meta-analysis

Akbari M, et al. - Among patients with gastric dysplasia (GD), researchers analyzed the incidence rate of gastric adenocarcinoma (GAC). Using random-effect or fixed-effect models, the incidence rates of GAC and their corresponding 95% confidence intervals (CIs) were pooled. Of the 1,980 retrieved records, 30 eligible articles (61 studies) have been involved. In patients with GD, the overall pooled incidence rate of GAC was 40.36 cases per 1,000 person-years. Subgroup analysis by type of GD indicated the highest incidence rate of GAC in patients with high-grade dysplasia (HGD) lesions was 186.40 per 1,000 person-years. While GAC incidence rates were 11.25 and 1.40, respectively, in low-grade dysplasia (LGD) lesions and non-classified lesions. According to this systematic review and meta-analysis, progression rate from GD to GAC was roughly 16 times greater in patients with HGD lesions vs those with LGD lesions. Since most GAC patients were diagnosed at an advanced stage, the study suggests strict control of HGD lesions in order to prevent GAC.
