Incidence, predictors, impact, and treatment of vascular complications following transcatheter aortic valve implantation in a modern prospective cohort under real conditions

Langouet Q, Martine R, Saint-Etienne C, et al. - Prior studies have frequently reported vascular complications (VCs) occurring in transcatheter aortic valve implantation (TAVI) procedures, researchers here examined the incidence, impact, and prognostic factors of VCs in a recent real-world cohort given the significant technical improvements in delivery systems and vascular closure devices. In this bicentric prospective analysis, vascular complications were assessed in 479 consecutive patients who underwent TAVI between January 2017 and December 2017. Criteria set out by the Valve Academic Research Consortium Criteria-2 were used define vascular complications. VC incidence of 26.1% was observed (n = 125 patients); of these, 2.9% were major (n = 14) and 23.2% were minor (n = 111). Occurrence of VCs was thus observed in more than one-quarter of TAVI procedures; these were primarily minor ones. One-third of VCs were observed in correlation with secondary puncture points and hence, these should also be actively monitored. Short and mid-term survival are significantly influenced by major VCs.
