Incidence, predictors and impact of stroke on mortality among patients with acute coronary syndromes following percutaneous coronary intervention—Results from the PROMETHEUS registry

Chandiramani R, et al. - Among acute coronary syndrome (ACS) patients (n=19,914) who underwent percutaneous intervention (PCI) in the PROMETHEUS multicenter observational study, the incidence, predictors as well as the influence of stroke on death was investigated. Within 1 year, a stroke occurred in 244 patients, resulting in a cumulative incidence of 1.5%. This implies that the development of stroke within 1 year among ACS patients undergoing PCI was not an uncommon finding. For stroke occurrence after PCI, the strongest predictor was previous cerebrovascular disease, followed by ST-elevation myocardial infarction (MI) presentation, hypertension, non-ST-elevation MI presentation, smoking, female sex, and age. Those who had a stroke had a significantly higher risk of death as compared to those who did not have a stroke. Compared with MI and bleeding, a substantial influence of stroke on death was evident that reduced rapidly over time.
