Incidence of severe hypoglycemia and possible associated factors in pediatric patients with type 1 diabetes in the real-life, post-Diabetes Control and Complications Trial setting: A systematic review

Cherubini V, et al. - In pediatric patients with type 1 diabetes (T1D), researchers intended to determine if the incidence of severe hypoglycemia (SH) has changed since 1993 in a real-world setting. A systematic PubMed literature search for prospective or retrospective observational studies (≥250 candidates) on SH epidemiology or related topics in pediatric T1D patients released between October 1993 and June 2016 recognized 35 articles (involving >55,000 candidates). Findings revealed that SH has decreased since the Diabetes Control and Complications Trial in pediatric patients with T1D, but it remains a prevalent issue. Factors that might have affected this temporal trend in SH incidence included the increased use of new types and methods of administering insulin, specifically rapid-acting insulin analogs and continuous subcutaneous insulin infusion. To help lower the risk of SH in this population, the ideal use of fresh insulin therapies/regimens/technologies, enhanced education, and dedicated specialized management teams are required.
