Incidence of new-onset left bundle branch block and predictors of new permanent pacemaker following transcatheter aortic valve replacement with the Portico valve

Walther T, et al. - Using the repositionable, self-expanding Portico™ transcatheter aortic valve implantation (TAVI) system (St. Jude Medical, St Paul, MN, USA), researchers examined the incidence and timing of new-onset left bundle branch block (LBBB) and new permanent pacemaker implantation (PPI) in patients undergoing TAVI. In addition, they assessed the clinical impact and periprocedural factors associated with new PPI. A prospective analysis was performed of a total of 198 high-risk patients without pre-existing pacemaker from the Pre-CE Mark Portico TAVI system study. Outcomes suggested an acceptable rate of new PPI at 1 year without increased mortality risk to patients with the Portico TAVI system. During the procedure, new-onset LBBB occurred commonly; the majority of events resolved spontaneously by 30 days. They identified no intraprocedural factors that may forecast new PPI independently.
