Incidence, factors and prognostic impact of re-exploration for bleedings after continuous flow left ventricular assist device implantation

Iwasaki K, Yoshitake K, Yagi N, et al. - Given a common occurrence of bleeding complications after continuous flow left ventricular device (CF-LVAD) implantation, and a requirement for a re-exploration for bleedings in some cases, researchers here examined the incidence, timing and risk factors for a re-exploration for bleedings after CF-LVAD implantation. Among 162 consecutive patients (age 43 ± 13 years, 71% men) who underwent CF-LVAD implantation (HeartMateII 119, Jarvik2000 15, HVAD 13, EVAHEART 10, DuraHeart 5) from January 2012 to June 2019, a re-exploration for bleedings was done in 35 (21.6%) patients. Findings suggest a prevalent need for a re-exploration for bleedings after CF-LVAD implantation, particularly in cases with low postoperative platelet counts. Multivariate logistic regression analysis revealed an independent predictive value of a postoperative platelet count for a re-exploration for bleedings after CF-LVAD implantation (per 104/μl: odds ratio 0.82). Given a poor prognosis in correlation with bleedings requiring a re-exploration, the risk stratification using postoperative platelet counts may be advantageous for cases who underwent CF-LVAD implantation.
