Incidence and risk factors of postoperative delirium in patients admitted to the ICU after elective intracranial surgery: A prospective cohort study

Wang CM, Huang HW, Wang YM, et al. - Researchers performed this prospective cohort study to define the incidence as well as risk factors of postoperative delirium (POD) in patients following intracranial surgery. This inquiry was held at a neurosurgical ICU of a university-affiliated hospital, Beijing, China. Consecutive enrollment of adult patients admitted to the ICU following elective intracranial surgery under general anaesthesia was done from 1 March 2017 to 2 February 2018. They used the Confusion Assessment Method for the ICU in order to evaluate delirium. This study included 800 patients, of whom, 157 received a diagnosis of POD. Age, nature of intracranial lesion, frontal approach craniotomy, duration of surgery, presence of an episode of low pulse oxygenation at ICU admission, presence of inadequate emergence and emergence delirium, postoperative pain and presence of immobilising events were all identified as the independent risk factors for POD. Adverse consequences and high costs were also reported in relation to POD. Overall, POD prevailed among patients following elective intracranial surgery. Experts recommended establishing a comprehensive strategy including screening for predisposing factors and early prevention of modifiable factors in this patient populace.
