Incidence and prognostic impact of atrial fibrillation after discharge following revascularization for significant left main coronary artery narrowing

Kosmidou I, Liu Y, Zhang Z, et al. - Among patients who underwent percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) for left main coronary artery disease (LMCAD), researchers examined the 3-year incidence as well as the clinical influence of post-discharge atrial fibrillation or flutter (AF). Randomization of 1,905 patients with LMCAD to PCI vs CABG was done in the EXCEL trial. The study cohort included overall 1,802 patients without AF at baseline. The occurrence of 227 episodes of AF was reported within 3 years among the participants (29 [12.8%] in the PCI arm and 198 [87.2%] in the CABG arm); of those, 63 (27.7%) happened after discharge from the index hospitalization in 57 patients. Findings revealed increased mortality and stroke in relation to post-discharge AF after CABG or PCI for LMCAD. AF after discharge was independently predicted by in-hospital atrial fibrillation.
