Incidence and etiology of microinfarcts in patients with ischemic stroke

Oliveira-Filho J, et al. - Experts sought to explore the incidence and potential causes of cerebral microinfarcts (CMI) linked with intracerebral hemorrhage due to small vessel disease (SVD). Researchers identified CMI (≤5 mm) on diffusion-weighted imaging in 28% of study participants, 71% within the vascular territory of the primary infarct. Presence of CMI was independently connected with large-vessel atherosclerosis, cardioembolic source, higher white matter hyperintensities (WMH) and lower systolic blood pressure. It was determined that CMIs occurring within the vascular territory of a larger infarct were mostly embolic, but those occurring outside could be associated with SVD. Study results indicate that SVD plays a role in the pathogenesis of CMIs and highlighted the significance of etiologic workup to detect alternate etiologies.
