In-hospital outcomes of transcatheter vs surgical aortic valve replacement for nonagenarians

Elgendy IY, et al. - Researchers used the National Inpatient Sample years 2012–2014, to compare in-hospital mortality and peri-procedural outcomes of transcatheter aortic valve replacement (TAVR) vs surgical aortic valve replacement (SAVR) in nonagenarians by using unadjusted, multivariate logistic regression, and propensity score matched analyses. A total of 6,680 records of nonagenarians undergoing aortic valve replacement, among which 5,840 (87.4%) underwent TAVR, were analyzed. In this nationwide analysis, an overall similar incidence of in-hospital mortality was observed with TAVR vs SAVR. A lower incidence of acute kidney injury, post-operative blood transfusion, a higher likelihood of discharge to home, and a similar incidence of pacemaker placement and stroke were reported in association with TAVR vs SAVR in the propensity-matched analysis. Overall, findings demonstrated efficacy as well as safety of TAVR in nonagenarians.
