Improving detection of multiple sclerosis lesions in the posterior fossa using an optimized 3D-FLAIR sequence at 3T

Lecler A, et al. - Through a prospective study of 27 subjects with confirmed multiple sclerosis (MS), experts evaluated the diagnostic value of optimized 3D-FLAIR in detecting infratentorial MS lesions in comparison with an axial T2-weighted imaging, a 3D-FLAIR with factory settings, and a 3D double inversion recovery sequence. Out of 27, 22 had at least 1 lesion in the posterior fossa. In contrast to any other sequence, the optimized FLAIR sequence identified significantly more posterior fossa lesions. The reading confidence index was significantly greater with the optimized FLAIR, and the contrast was significantly higher with the optimized FLAIR as compared with the standard FLAIR and the double inversion recovery. Hence, in patients with MS, an optimized 3D-FLAIR sequence was observed to enhance posterior fossa lesion detection.
