Improved RA classification among early arthritis patients with the concordant presence of three RA autoantibodies: Analysis in two early arthritis clinics

Regueiro C, Rodríguez-Martínez L, Nuño L, et al. - In this analysis done in two early arthritis (EA) clinics, data from the first visit of 1,057 individuals in two EA prospective Spanish cohorts were utilized in order to assess the potential of the concordant presence of rheumatoid factor (RF), anti-CCP and anti-carbamylated protein antibodies to enhance prevailing RA classification among people with EA. Predictive characteristics allowing for direct classification as RA were exhibited by the concordant presence of three antibodies and were notably superior to the corresponding to the high antibody titers described as in the 2010 classification criteria. The concordant presence of two antibodies was also very enlightening. Results permitted devising a scoring system on the basis of only antibody concordance that displayed comparable overall performance as the serological scoring system of the 2010 criteria. Nonetheless, by combining the concordance and 2010 serological systems, the best classification was achieved, a combination with a notable contribution from each of the two systems. In summary, the concordant presence of RA autoantibodies exhibited an independent addition to the classification of individuals with EA that allowed heightened differentiation and accuracy.
