Importance of preoperative ultrasound examination and pathological tumour evaluation in the management of women with endometrial cancer

Sobocan M, Ogrizek AM, Ledinek T, et al. - Researchers performed this single-centre retrospective study to determine if a diagnostic approach consisting of pre-operative imaging with transvaginal ultrasound (TVUS) and histological endometrial biopsy assessment is accurate for patient management decisions for cases of endometrial cancer (EC). All patients with EC managed between 2015 and 2019 were included herein. TVUS assessment of myometrial invasion was found to be 76% sensitive and 81.7% specific. A higher diagnostic accuracy was obtained for Type I EC (95%) vs Type II EC (54%). For predicting lymph node involvement, the significance of only presumed ESMO/ESGO/ESTRO risk classification and deep myometrial invasion was evident. Overall, a moderately accurate evaluation of the risk of EC was allowed by pre-operative TVUS examination and pathological endometrial biopsy assessment. To enhance the preoperative evaluation of risk in patients with EC, attempts should be directed towards the creation of new and more reproducible methods, such as molecular tumour characterization.
