Impact on child acute malnutrition of integrating a preventive nutrition package into facility-based screening for acute malnutrition during well-baby consultation: A cluster-randomized controlled trial in Burkina Faso
PLoS Medicine Sep 10, 2019
Becquey E, Huybregts L, Zongrone A, et al. - Sixteen health centers were randomized to the intervention arm and were compared with 16 health centers that offered the usual screening services in order to check the hypothesis that the integration of a preventive package comprising age-appropriate behavior change communication (BCC) on nutrition, health, and hygiene practices and a monthly supply of small-quantity lipid-based nutrient supplements to the monthly screening would raise acute malnutrition (AM) screening and treatment coverage and reduce the incidence and prevalence of AM. In the intervention in contrast with the comparison arm, the proportion of children screened for AM was 18–23 percentage points greater. Nevertheless, no influence on the proportion of children who received relevant treatment for AM or in the prevalence or incidence of AM was discovered. Therefore, adding a preventive package to community management of acute malnutrition (CMAM) delivered at health facilities in Burkina Faso rose participation in monthly AM screening, hence, overcoming a major obstacle to CMAM efficiency. Moreover, the lack of influence on AM treatment coverage and on AM prevalence and incidence directs research to discuss the residual barriers to uptake of preventive and treatment services at the health center and to recognize and test complementary strategies to bring integrated preventive and CMAM services nearer to the community while assuring a high-quality implementation and service delivery.
