Impact of the new pulmonary hypertension definition on heart transplant outcomes: Expanding the hemodynamic risk profile

Crawford TC, Leary PJ, Fraser CD, et al. - Given that pulmonary hypertension (PH) definition was defined again at the recent 6th World Symposium on Pulmonary Hypertension to include lower pulmonary artery pressures in the setting of high pulmonary vascular resistance (PVR), researchers investigated the significance of this change to individuals with PH because of left-heart disease as well as the preoperative evaluation of heart transplant (HT) recipients. Participants were adult recipients who had primary HT from 1996 to 2015. These individuals were identified from the United Network for Organ Sharing database. The subdivision of recipients into those with mean pulmonary artery pressure < 25 mm Hg and ≥ 25 mm Hg was done. Findings revealed the link of elevated PVR with a significant rise in the hazard for 30-day mortality following cardiac transplantation, even in the setting of lower pulmonary artery pressures. The validity of the novel definition of PH was supported.
