Impact of surgical approach on oncologic outcomes in women undergoing radical hysterectomy for cervical cancer

Cusimano MC, Baxter NN, Gien LT, et al. - Patients undergoing minimally invasive vs those undergoing open radical hysterectomy for cervical cancer were examined for oncologic outcomes in correlation to surgical approach while accounting for mechanistic factors including surgeon volume. For this population-based retrospective cohort study, 958 patients (minimally invasive 475; open 483) with mean age 45.9 and a median follow-up of 6 years, were identified. Patients with stage IB cervical cancer displayed increased rates of death and recurrence in relation to minimally invasive radical hysterectomy even after controlling for surgeon volume; hence this population should undergo an open radical hysterectomy. Although minimally invasive radical hysterectomy seemed to be safe for a subset of patients with microscopic early-stage disease, additional studies are recommended.
