Impact of radiation therapy dose escalation on prostate cancer outcomes and toxicities

Zaorsky NG, et al. - In this meta-analysis of prostate cancer (PCa) patients from randomized-controlled trials of external beam radiation therapy (RT), researchers looked at the correlation of increasing the PCa biologically equivalent dose (BED) of external RT with freedom from biochemical failure (FFBF) and overall patient outcomes: overall survival (OS), distant metastasis (DM), and cancer-specific mortality (CSM); as well as genitourinary (GU), and gastrointestinal toxicities. Consequent to RT BED escalation, significantly improved PCa FFBF at up to 10 years without improved OS, DM, or CSM was noted. FFBF is a poor surrogate of overall patient outcomes for trials of RT. Even at higher BED, intensity-modulated RT (IMRT) vs 3-dimensional conformal RT (3D-CRT) caused less frequent late toxicities.
