Impact of preoperative platelet count on bleeding risk and allogeneic transfusion in multilevel spine surgery

Chow JH, Chancer Z, Mazzeffi MA, et al. - This study was sought to distinguish which individuals are at high risk for allogeneic transfusion which may allow for better preoperative planning and employment of specific blood management strategies. Researchers applied a univariate logistic regression model to distinguish variables that were significantly correlated with intraoperative allogeneic transfusion. The adjusted association of these variables with intraoperative transfusion was measured by using a multivariate forward stepwise logistic regression model. They conducted multilevel thoracic and lumbar spine surgery in 921 patients. The data exhibited that in multilevel thoracic lumbar spine surgery, the preoperative thrombocytopenia significantly contributes to intraoperative transfusion. Distinguishing factors that may elevate the risk for transfusion could be of great advantage in better preoperative counseling of individuals and in decreasing overall cost and postoperative complications by implementing strategies and techniques to decrease blood loss and blood transfusions.
