Impact of plaque dilation before carotid artery stent deployment

Lauricella A, et al. - Researchers conducted this prospective nonrandomized multicenter study to investigate the impact of maximum dilation of the carotid plaque before stent deployment (max-pre-SD) or after stent deployment (post-SD) on macroscopic plaque debris, hemodynamic depression (HD), and immediate major adverse events. Transcranial Doppler evaluated and analyzed the number of microembolic signals (MESs). A total of 309 patients have been recruited and treated using a proximal cerebral embolic protection device with standard CAS performed. In 100% of cases, technical success has been achieved. Findings suggested that max-pre-SD appears to be a safe and feasible technical modification to the CAS procedure. Compared with CAS with post-SD, macroscopic debris, HD, and MESs are significantly decreased. To determine the superiority of this technical modification, further research with larger, randomized patient cohorts is needed.
