Impact of patient's global assessment on achieving remission in patients with rheumatoid arthritis: A multinational study using the METEOR database

Ferreira RJO, Carvalho PD, Ndosi M, et al. - Cross-sectional data from the Measurement of Efficacy of Treatment in the Era of Outcome in Rheumatology international database were utilized in order to ascertain the impact of patient's global assessment (PtGA) on RA disease states, investigate variations across countries, and learn the relationship between PtGA, measures of disease influence (symptoms), and markers of disease activity (inflammation). This study involved 27,768 individuals. The remission rate rose from 5.8% to 15.8% by excluding PtGA from the Boolean-based definition (3-variable remission). The rate of PtGA-near-remission differed significantly between countries, ie, from 1.7% in India to 17.9% in Portugal. In the PtGA-near-remission group, one-third of the individuals scored PtGA > 4 of 10. With inflammation-related variables contributing less to the model, the main correlates of PtGA were pain and function. Hence, PtGA is moderately associated with joint inflammation overall, although only faintly so in low levels of disease activity. Moreover, placing them at risk of excessive immunosuppressive treatment, a substantial proportion of individuals otherwise in biologic remission still notice high PtGA.
