Impact of obstructive sleep apnea on the obesity paradox in critically ill patients

Bailly S, Galerneau LM, Ruckly S, et al. - By analyzing retrospectively gathered data from adult patients admitted to ICU at two university hospitals, researchers determined how obstructive sleep apnea (OSA) can influence the prognosis of ICU patients. The outcomes of interest were length of stay in the ICU, ICU mortality, in-hospital mortality, ventilator-associated pneumonia (VAP). This study involved 5,146 patients, including 289 having OSA at ICU admission (5.6%). OSA significantly influenced the length of ICU stay for patients having BMI over 40 kg/m2, as revealed in a predefined subgroup analysis. Experts found no influence of OSA status on ICU or hospital mortality and VAP. In general, ICU stay was not increased by known OSA except for cases with both OSA and morbid obesity.
