Impact of ischaemic heart disease severity and age on risk of cardiovascular outcome in diabetes patients in Sweden: A nationwide observational study
BMJ Open Apr 08, 2019
Jernberg T, et al. - Using a highly representative nationwide sample of all patients with type 2 diabetes (T2D) initiating first-time glucose-lowering drug (GLD) in Sweden over 7 years, researchers compared short-term (3-year) cardiovascular (CV) outcome in these patients without ischemic heart disease (IHD), with IHD but no prior myocardial infarction (MI), and those with prior myocardial infarction (MI). They also evaluated the impact of age when initiating first-time GLD. Patients with T2D but not IHD had a lower CV outcome risk vs those with T2D and IHD, driven primarily by a higher MI risk. An almost linear association was observed between age at start of GLD and relative risk for T2D patients without IHD whereas, in IHD patients, younger patients had a relative greater risk vs older patients. The key strategy for managing T2D patients, especially younger patients, may be intensive risk prevention.
