Impact of histological components on selecting limited lymphadenectomy for lung adenocarcinoma ≤ 2 cm

Sun W, Su H, Liu J, et al. - This analysis of 1,160 surgical patients with invasive lung adenocarcinoma ≤ 2 cm was conducted to identify patients for whom limited mediastinal lymphadenectomy (LML) can be beneficial based on the percentage of histological components (PHC). This study included 882 patients with systematic mediastinal lymphadenectomy (SML) and 278 with LML. An independent link of LML, compared to SML, with worse prognosis was shown for patients with PHC MIP+S ( total percentage of micropapillary and solid components) > 5 % but not for those with PHC MIP+S ≤ 5 %, as shown in multivariable Cox analysis. Experts concluded that PHC MIP+S has predictive worth for N2 metastasis as well as procedure-specific outcome (LML vs SML). They also suggested its likely utility as a feasible indicator for detecting appropriate candidates of LML by using intraoperative frozen section.
