de Miguel-Diez J, et al. - Using the Spanish National Hospital Discharge Database (2014–2015), researchers examined hospitalized patients with and without COPD to determine the use of transcatheter aortic valve implantation (TAVI) and conventional surgery (SAVR) in these subjects. They also compared patients with COPD undergoing TAVI and SAVR in terms of in-hospital mortality (IHM), length of hospital stay (LOHS) and cost. They also investigated the factors related to IHM among these patients. Overall, 2,141 patients who had TAVI (27.60% with COPD) and 16,013 patients who had received SAVR (19.31% with COPD) were included. Compared to non-COPD patients, a worse prognosis during hospitalization was not observed in COPD patients undergoing TAVI. However, significantly higher mortality was observed for SAVR in patients with COPD vs patients without this condition. Compared with propensity score matched TAVI patients, higher IHM and LOHS were demonstrated by COPD patients who underwent SAVR.
