Impact of COPD on outcomes in hospitalized patients treated with transcatheter aortic valve implantation or surgical aortic valve replacement in Spain

de Miguel-Diez J, et al. - Utilizing the Spanish National Hospital Discharge Database (2014–2015), researchers studied hospitalized patients with and without COPD to determine the use of transcatheter aortic valve implantation (TAVI) and conventional surgery (SAVR) in these individuals. In addition, they contrasted patients with COPD undergoing TAVI and SAVR in terms of in-hospital mortality (IHM), length of hospital stay (LOHS) and cost. They also examined the factors related to IHM among these patients. Overall, 2,141 patients who had TAVI (27.60% with COPD) and 16,013 patients who had received SAVR (19.31% with COPD) were involved. Compared with non-COPD patients, a worse prognosis during hospitalization was not observed in COPD patients undergoing TAVI. However, significantly higher mortality was noted for SAVR in patients with COPD vs patients without this condition. Compared with propensity score-matched TAVI patients, higher IHM and LOHS were shown by COPD patients who underwent SAVR.
