Impact of chronic total occlusion and revascularization strategy in patients with infarct-related cardiogenic shock: A subanalysis of the CULPRIT-SHOCK trial

Braik N, Guedeney P, Behnes M, et al. - Researchers investigated the effect of coronary artery chronic total occlusion (CTO) as well as its treatment with percutaneous coronary intervention (PCI) in the setting of myocardial infarction (MI) related cardiogenic shock (CS), in this pre-specified analysis from the The Culprit Lesion Only PCI versus Multivessel PCI in Cardiogenic Shock (CULPRIT-SHOCK) trial. Patients with MI and multivessel disease complicated by CS were randomly assigned to a culprit-lesion-only or immediate multivessel PCI strategy in the CULPRIT-SHOCK trial. Multivariate logistics models were employed to assess the independent links between the presence of CTO and adverse results at 30 days as well as at one year. According to findings, the presence of CTO was identified to be related to adverse outcomes in patients with MI-related CS and multivessel disease, while culprit-lesion-only PCI strategy appeared advantageous irrespective of the presence of CTO.
