Impact of a policy to deliver at 39 weeks for the indication of class III obesity

Pierce SL, Maxted ME, Peck JD, et al. - Researchers conducted this retrospective cohort study to assess the impact of a policy to deliver at 39 weeks for class III obesity. The sample consisted of women with class III obesity delivering at ≥ 37 weeks before (May 2012 to April 2014) and after the policy (September 2014 to August 2016). Participants in the study were 1,210 patients (580 before the policy and 630 after the policy). Cesarean rates were comparable before and after the policy. Parous women had lower cesarean rates but nulliparous women had higher cesarean rates after the policy in adjusted comparisons of women undergoing labor induction. Between groups, rates of chorioamnionitis, endometritis, and cesarean wound infection were not different. Between pre- and postpolicy groups, composite neonatal morbidity was not different. A policy of delivery at 39 weeks for class III obesity had no effect on overall cesarean rate or rates of maternal or neonatal morbidity.
