• Preinvasive lesions were obtained from the surgical pathology laboratory.

• Immune infiltrate abundance was evident in proximal colon lesions, paralleling the responses noted in normal mucosa and invasive disease.

• Increasing villous histology was associated with diminishing immune response.

• There were no differences in lesions with low- and high-grade dysplasia, except in mast cells.
