Hysterotomy level at cesarean section and occurrence of large scar defects: A randomized single-blind trial

Vikhareva O, et al. - Researchers performed a two-center, randomized, single-blind trial of a surgical procedure to examine the effect of cesarean hysterotomy level on the occurrence of large uterine scar defects 6-9 months after delivery. They randomized women in active labor (cervix dilatation ≥5 cm) randomized to a high incision (HIGH) or a low incision (LOW) group and performed hysterotomy 2 cm above (HIGH) or 2 cm below (LOW) the plica vesicouterina. In the high incision group, large scar defects were seen in 4 of 55 women vs 24 of 59 women in the low incision group. Outcomes thus suggest the existence of the association between the level cesarean hysterotomy and the occurrence of large scar defects detected by transvaginal ultrasound 6-9 months after delivery.
