Hysterectomy with sentinel lymph node biopsy in the setting of pre-operative diagnosis of endometrial intraepithelial neoplasia: A cost-effectiveness analysis

Lim SL, et al. - For a pre-operative diagnosis of endometrial intraepithelial neoplasia (EIN), researchers evaluated how cost-effective different surgical management strategies were. The four surgical strategies compared were hysterectomy, hysterectomy with frozen section (hysterectomy+frozen), hysterectomy with sentinel lymph node biopsy (SLNB) (hysterectomy+SLNB), and hysterectomy with frozen section and SLNB (hysterectomy+frozen+SLNB). From a third-party payer perspective, they created a decision model. They observed limited clinical benefit and greater costs associated with hysterectomy+frozen+SLNB vs hysterectomy+frozen for pre-operative EIN. Based on the results, standard of care in these patients should continue to be hysterectomy+frozen and subsequent intraoperative staging decisions.
