Human papillomavirus–associated oropharyngeal cancer: Patterns of nodal disease

Plonowska KA, et al. - In human papillomavirus (HPV)–associated oropharyngeal squamous cell carcinoma, represented by p16 positivity (p16+OPSCC), authors described patterns of neck lymph node (LN) metastases. This study was a case series with chart review of neck dissection (ND) specimens from nonirradiated p16+OPSCC patients. Any levels bearing suspicious LNs and levels IIA-B, III, and IV should be encompassed by therapeutic NDs for p16+OPSCC, while elective NDs should be performed and encompass at least levels IIA-B and III. Findings suggested an association of these selective ND plans, followed by indicated adjuvant treatment, with a low nodal recurrence rate.
