How useful are hemoglobin concentration and its variations to predict significant hemorrhage in the early phase of trauma?: A multicentric cohort study

Annals of Intensive Care Jul 13, 2018

Figueiredo S, et al. - In this analysis of all consecutive adult trauma patients directly admitted to 6 French level-1 trauma centers with at least one prehospital Hb measurement, researchers evaluated the abilities of hemoglobin (Hb) measurements taken at different times throughout trauma management to identify patients with significant hemorrhage (SH). They found that significant hemorrhage was predicted by the first Hb measurements taken using a point-of-care (POC) device on scene (POC-Hbprehosp), albeit modestly. This finding challenged the classical idea that early Hb measurement is not meaningful in predicting SH. Relative to shock index, a greater predictive ability for SH was demonstrated by Hb as measured with POC device upon patient’s admission to the hospital (POC-Hbhosp). In patients with significant hemorrhage vs in controls, significantly higher magnitude of the Hb drop was observed for a given prehospital fluid volume infused.
