Hospital and surgeon reimbursement trends for femoral neck fractures treated with hip hemiarthroplasty and total hip arthroplasty

Quinlan ND, Hogarth DA, Chen DQ, et al. - This research was attempted to explore the trend in hospital charges and payments relative to surgeon charges and payments in a Medicare population for hip hemiarthroplasty and total hip arthroplasty (THA) for femoral neck fracture. Between 2005 and 2014, researchers applied the 5% Medicare sample database to capture hospital and surgeon charges and payments related to 32,340 patients who had undergone hemiarthroplasty and 4,323 patients who had undergone THA for femoral neck fractures. They estimated two values: charge multiplier (CM, ratio of hospital to surgeon charges), and payment multiplier (PM, ratio of hospital to surgeon payments). They further analyzed year-to-year variation and regional trends in patient demographics, Charlson Comorbidity Index, length of stay, 90-day and 1-year mortality, CM, and PM. The results of this study indicate that hospital charges and payments relative to surgeon charges and payments have elevated substantially for hemiarthroplasty and THA conducted for femoral neck fractures.
